Completed application form (including health questionnaire) & clinical skills questionnaire
Enhanced CRB clearance
Passport & proof of eligibility to work in the UK
2 Clinical references
Evidence of professional qualifications
CV
National Insurance Number (unless you have not yet worked in the UK)

For certain specialities you will also need to provide an up to date:

Immunisation Record (Hep B)
HPC (Health Professions Council) Registration
